1). Enter the Shareholders according to the formula, entering them in separate fields. For each Shareholder, provide the value of shares expressed in percentages. Example: "NAME(S) LAST NAME – X %";

a. if one Shareholder – example: "ANTHONY BRIAN SMITH – 100%",

b. if there are more Shareholders, enter each of them in a separate field – example: "SUZANNE MARIE JOHNSON – 70%", "CHRISTOPHER HAWKINS – 30%".

ATTENTION:
- Please note that the sum of all shares must be 100%.
- If there are more Partners than the number of fields in the form, more of them should be added in the message ("ADDITIONAL INFORMATION / COMMENTS" – form below).
